Register with a GP
Find practices near your new home through NHS 111 Wales. Contact your chosen practice to check its catchment and whether it can register you, then follow its registration process.
Have your previous GP details and NHS number available if known. Ask the practice what information it needs, how your records will transfer and how to arrange any continuing care. If you have difficulty finding a practice, follow the support route in the NHS Wales GP registration guidance.
Catchment is about your address
A town name, postcode district, council area or nearby hospital does not establish whether a practice can register you. NHS Wales Shared Services advises contacting the practice directly to confirm that it covers your address. A branch surgery is part of a practice, not necessarily a separate registration choice.
Use a practice’s published boundary checker where available, then confirm uncertain addresses with reception. Ask which site you would attend, how home visits and out-of-hours care are arranged, and how planned reviews or prescriptions will continue. If you are staying temporarily, ask about temporary registration.
Do not enter your full address in this website or Christine’s chat. Give it directly to the practice through its own contact or registration process.
Medicines and ongoing appointments
Before moving, discuss continuity of prescribed medicines and any planned reviews with your existing healthcare team. Keep an up-to-date medicine list and details of outstanding appointments or investigations.
Find a convenient pharmacy and check its opening hours. For a dentist, use the NHS Wales directory and ask directly about NHS availability; a directory listing does not guarantee an appointment.
Finding the right service
Call 999 for a life-threatening emergency. NHS 111 provides urgent advice and access to GP out-of-hours services in Wales. This relocation guide is not a clinical assessment or emergency service.
A GP practice catchment does not decide every hospital referral. Read the cross-border care guide to understand planned, urgent and specialist pathways.
